Oak Healthy Living Centre Covid -19 Health Declaration

To help prevent the spread of COVID -19 in the Oak Healthy Living Centre, every participant/parent/guardian/volunteer/visitor must complete and sign a Health Declaration and sign Track & Trace record on entrance to the building .

Please do not come to the building if you are displaying any COVID symptoms, or are in close contact with anyone with COVID.

Please wear a face covering. Temperatures will be taken on entrance to the building by staff. Please maintain social distancing and use hand sanitizer stations available throughout the building.

On review of the forms completed . The Oak Healthy Living Centre Management may contact you and ask you to go home and self Isolate .

If you have symptoms, contact your GP or Out Of Hours Service.
